Absolutely! For current patients, please call or email to request this. The estimate will be sent within 10 business days (less for emailed information).
Charges for appointments are based on the complexity of your child's diagnosis and/or medications, as well as the total time spent for completing the necessary responsibilities for your child's care on the day of their appointment, as well as the time for the appointment itself.
**It is also important to know what your child's medical insurance requires for your out-of-pocket costs, for example, a copay. Contact your health plan for more information.
**If your child's health insurance plan does not have mental health coverage, NO PROBLEM! The charges for my services are processed as medical claims, like a sick visit at your child's primary care clinic.
Please note: In a situation where this request could delay care, during which time your child could to run out of medication. However, my priority is to try to prevent this happening by providing a "bridge" Rx with enough medication to last until the next appointment.
Yes, while PMHP is pending enrollment in several insurance plans (see up-to-date listing on home page), I recognize that this is a significant factor to be able to obtain medical care for your child.
Please call during regular business hours to discuss payment plans and discounts on self-pay rates until your child's insurance is accepted.
**If your child does not have any medical insurance coverage: PMHP is not a "federally-qualified health center" (FRSA), meaning that it does not receive federal funding in order to provide a "sliding scale fee" based on household size and income. However, I will consider each family's individual circumstances in order to provide a very reasonable discount or payment plan.
That Depends...Maybe
**All Payments or Copays are due PRIOR to the APPOINTMENT!
These payments can be made by a few different methods. A Link will be sent via text from Cardpointe and your credit card information can be saved (as an encrypted token, the real number is never seen by me). Once a credit card is saved in Cardpointe, I can also set up an individual auto-bill to charge your card on the date that the next appointment is scheduled (one appointment at a time, unless otherwise discussed in advance).
However, If your insurance plan denies the claim due to "deductible not met," or another member-based reason, then the financial guarantor for the child will be liable for the remaining balance. Since insurance plans vary in how quickly they process individual claims, (from 2 weeks to 2-3 months after the appointment), there will not be late charges or other penalties added on to this balance at that time. If this balance is not paid off within 30 days, (unless otherwise discussed in advance), then late fees may be charged. You will be notified about the amount of these late fees before they are added to the account balance.
Yes. Certain extra fees such as FMLA paperwork and any applicable late fees for outstanding balances are not billable to insurance companies. Any fees charged for frequent No-Shows or late (same-day) cancellations to appointments are also not billed to insurance. The financial guarantor will be responsible for any balance and fees on the account.
If you are relocating and need a copy of your child's medical records, there is a federal/state regulated standard maximum charge of $25 (<500 pages, when sent electronically directly to you).
**However, if you are requesting medical records to be sent directly to your child's pediatrician, or other medical provider, you will not be charged for this service, since I believe that the continuity of your child's care plan is vitally important for their mental and physical health!
